We tried below steps to make it work.

1- Stop all your servers

2- Remove the offending entry from the config.xml file, if it is still there. It could be something similar to this:

<app-deployment>
<name>TpStatus</name>
<target>Cluster1</target>
<module-type>ear</module-type>
<source-path>scratch\TpStatus-342557.ear</source-path>
<plan-path>D:\app\user_projects\domains\apps\config\plan\Plan-TpStatus-342557.ear.xml</plan-path>
<security-dd-model>DDOnly</security-dd-model>
<staging-mode>stage</staging-mode>
</app-deployment>

3- Restart all the servers(Clear cache and tmp folders)

4- copy your ear/war from its original location (to avoid the possibility of a trashed file)

5- deploy once again the app in the WLS console (activating the change, and then try to start the application in ACTIVE state)
